What are the three pieces of a three piece suit?

Does the shirt count as a piece, or is it separate, and the suit only includes the jacket, pants, and tie?

This has always confused me, because when I think about what you have to put on to wear a suit, I always come up with at least four items: dress shirt, pants, jacket, and tie.
